What is Family Transitional Nursing Care?


Family Transitional Nursing Care (FTNC) bridges the gap between hospital care and home transition for families welcoming a new baby. FTNC encompasses guidance, education, and emotional support to help all family members adjust to their new roles and responsibilities.


Who Can Benefit?


- First-time parents

- Families with multiples (twins, triplets, etc.)

- Families with newborns or mothers deemed to have high risk medical needs.

- Parents seeking extra support and reassurance during the early weeks at home.


What are the Objectives/ Goals of FTNC?


- Facilitate a smooth transition from hospital to home

- Build parental confidence in caring for newborns

- Promote maternal well-being and recovery

- Encourage partner involvement and support

- Foster a supportive and informed family environment.


My Care Philosophy:


Your new parenthood journey can be exciting, joyful, tiring, and downright overwhelming, all at the same time.


As a Registered Professional Nurse, I specialize in the care of low- and high-risk mothers and infants. In addition, I provide education and support to new fathers & partners. I support the family unit as they transition from the hospital to the comfort of their home, starting Day 1. Like Mary Poppins, when I depart my families, I know they are well equipped to navigate & assume total care w/ confidence and a decreased level of anxiety.


All Family Members Can Benefit:


For Newborns- provide education on feedings, comfort/soothing techniques, daily care, sleep habits, and safety (what to do in case of emergency).


For Mothers- continuation of Postpartum self-care (perineal or C-section incisional care); identify signs /symptoms of infection or postpartum complications; pain management tips; feeding preference support & education; breast care; Postpartum depression vs Baby Blues.


For Partner/Fathers- How to support your partner post birth; identifying signs and symptoms of possible Postpartum depression; Self-care/ support as a new parent; Reinforced education of  Newborn care (including feedings, comfort measures, etc).


Additional Services:


- Assistance with community resources and referrals (lactation consultants, mental health support, pediatricians, etc.)

- Phone call / text or virtual support available during contracted assignments.

- Customizable care plans tailored to each family's needs
